Who should get fined? For much of this there is currently no technological solution. Launch vehicles remain in a low orbit until they decay, satellites that fail can't be moved, and even those that end their life gracefully usually aren't deorbited because of cost constraints. The biggest problem isn't the satellites, but debris, and that's very difficult to control.

This isn't a problem caused by negligence of operators, and fines won't change anything. It's something that needs to be addressed by international policy and by advances in technology.

The vast majority of this stuff can't be deorbited, because it is uncontrolled. Also, only a tiny fraction of these objects are large enough to reach the ground. In fact, we count on natural decays to help keep the low Earth orbit environment from getting too filled up. Many objects (especially rocket bodies) only have a short lifetime in orbit.
